Facial wrinkles are divided into two:

Dynamic wrinkles are formed by the repetitive movement of underlying facial muscles. Forehead lines, crow's feet, wrinkles in the middle of the forehead are the most known examples.

Static wrinkles exist when the facial muscles are at rest. They occur with the effect of gravity and the deterioration of the skin / subcutaneous collagen structure. Nasolabial fold is an example.

Botox is an ideal option for dynamic facial wrinkles for patients older than 18. Mid 20's are excellent to begin whereas most people start using Botox at age 30. Early beginning age prevents wrinkles getting deeper. Deep wrinkles  can not be corrected easily, so PRP, stem cell injections, fat grafting and  fillers are usually combined with Botox .

 

Static lines require fillers, fat grafts or surgical interventions for correction.

Facial anatomy is complex, numerous tiny muscles are effective on our mimics. Even a simplest mimic is performed with perfect harmony of a large number of muscles (more than 20 to smile!). Botox application by somenone who does not know the location, depth and functional of each of these tiny muscles can cause a disaster.

Therefore, this is an important intervention that should be performed by a qualified plastic surgeon who knows functional anatomy. 

The number of injections varies from person to person. As wrinkles and depths increase, they will increase in number and dosage. Not only the number of injections to be made, but also the regions to be administered and the doses of application vary individually.

The main indications of Botox on the face are:

  • Forehead wrinkles

  • Frown lines

  • Nasal root lines

  • Eyebrow area - is done for the purpose of slightly lifting eyebrows.

  • Periorbital lines - Bunny lines on side ofthe nose of the eye and crow's feet etched in the corner of the eye.

  • Chewing - Masseter Muscle - Especially in those who have teeth geinding or who want a slimmer face.

Periorbital - crow's feet and bunny lines

Regions that are used less frequently but can be effective:

  • Lower and Upper Lip lines (Smoker's or Barcode lines) : While Botox can solve tiny lines alone, it is recommended to use fillers, stems cells, PRP and/or fat grafts in severe cases. In case of excess dosage, Botox may cause weakness in mouth movements, so it should be done extremely carefully.

  • Neck Lines (Neck Lift- Nefertiti Lift): Transverse lines develop due to prominent neck bands (platysmal bands), especially in underweight individuals. In people with good skin tension, better defined jaw line and tightened jowls with a mooth elongated can be achieved

  • Nasal Tip Lifting: Botox application to nose can lift the nasal tip and improve hook nose appearence to some degree.

  • Gummy Smile: With Botox to be injectedon the edge of the nose can reduce gingival exposurer ofr an attractive smile.

  • Nose-cheek fold (nasolabial fold or cheek fold): Intervention to the facial muscles involved in the fold fromation can seriously disrupt the facial expressions. For this area botox is contraindicated. Fillers, fat grafts or surgery in advanced cases may all be used.

  • Chin Crease and Puckering: Wrinkles and puckers due to mentalis muscle over activity respond very well to treatment

  • Marionette Lines: Expression lines coming down from the corner of the mouth cause an unhappy look. These are an ideal target for Botox treatment. Fillers may be used in conjuct.

Injections are to be made are made with very tiny needles (31-32 G) specially produced for Botox applications. Needles that are extremely thin are not felt in injection, but the drug given under the skin may cause mild burning or strecthing sensation. In patients who pain threshold is low, topical lidocain creams with ice application reduces pain during the injection.
